As for Vlaakith, go meet the captain of the place. There, several options are available to you. The first is to show him the relic, this will have no influence on the course of events. But you can also try to lie to him so as not to reveal to him that you have the relic.
Regardless, from the moment the captain learns that you have the relic, he will demand that you show it to him. If you refuse, he becomes hostile and you will have to defeat him to access the rest of the events. Same thing further when you meet the Inquisitor.
He will want to see the relic and you can either grant his request or refuse and fight him. Do as you wish, neither of these two choices will have a major impact on the sequence of events. Once the encounter is over, Vlaakith appears.
Should we challenge or help Vlaakith?
The latter then reveals to you that the relic that you have in your possession and which protects you from the Illithid infection is in fact his and that it has been corrupted by your Nocturnal Visitor, she then asks you to go inside to kill him.
Quite surprisingly, for once, Baldur's Gate 3 doesn't really give us a choice. Indeed, if you want to be smart about Vlaakith and send him to pasture, she will take it very badly and quickly make you understand that she is a goddess.
It's simple, if you refuse to help her, she will kill you and it's game over, pure and simple. Then agree to help him, without showing too much enthusiasm: many of your companions would not approveapart from Lae'Zel of course.
Should we kill our Nocturnal Visitor?
We just told you, you cannot refuse to help Vlaakith or at least, you cannot refuse to enter the relic. However, that doesn't mean you can't betray her once inside.
This is exactly what we will advise you to do. Indeed, once inside the relic, choose not to kill your Night Visitor and speak with him about the whole story. When you come out, Vlaakith will no longer be there.
On the other hand, the rest of the nursery and in particular the Inquisitor if you had not killed him before are not necessarily going to like the fact that you betrayed their queen and all these little people will become hostile towards you, forcing you to leave the nursery with great blows of your swords.
Should we take Lae'Zel to the Githyanki Nursery?
There you go, you now have everything about the meeting with Vlaakith, but there is still one point to discuss, that of Lae'Zel. As you can imagine, the choices you are going to make will directly concern her, the latter being extremely loyal to the queen of her people.
You therefore suspect that betraying Vlaakith may not go down well with her. Likewise, if you don't respect the latter during your conversation beforehand, she will take it rather badly. We therefore advise you to leave it at the camp during this passage of the game.
This will prevent you from ruining your relationship with her, knowing that it is entirely possible that she will decide to leave your team if you are not sufficiently linked and you fail your rolls during your discussion after choosing to betray Vlaakith.

In summary, you should obey Vlaakith's command to enter the Prism. At this stage, disobeying Vlaakith by refusing or asking her to do it herself or showing her too much disrespect will cause her to cast a spell that kills the whole party instantly. This leads to a Game Over screen.
After defeating them, Vlaakith will appear. If you agree to give the artifact to the Inquisitor, Vlaakith will demand you kill your Guardian. It is important to note that Lae'zel will go against you and even try to kill you if you do not go with the Inquisitor and Vlaakith's orders.
It's crucial to heed Vlaakith's command at this juncture, as defiance or disrespect could spell instant doom for your party, especially in the unforgiving Honor Mode. If you choose to wave politely to Vlaakith, you'll get a funny cutscene. Don't do this in Honor Mode.
Players should prioritize completing the Githyanki Crèche questline in Act 1, as it provides important information and tough enemies. Engaging in Lae'zel's companion quest can increase approval with her and help in choosing her as a romance option, but it may impact approval with other party members.
For a millennia old lich, even one as powerful as Vlaakith, that aging could destroy her body without proper preparations in advance. Also, as powerful as she is, she is not a goddess. She's not a god. She's a lich with dreams of godhood absorbing the souls of her followers trying to propel herself there.
Vlaakith makes an appearance as one of the overarching antagonists of the 2023 video game Baldur's Gate 3 (alongside the Dead Three, the Dark Urge, and Shar), serving as the main antagonist of Lae'zel's storyline.
If you deny Vlaakith order to kill the Spirit Guardian inside the Artifact (or Orpheus since it's what she actually mean), Vlaakith uses Wish Spell and ending our game (Wiping your party).

This ends up being a non-decision as far as the long-term story goes, but that hardly means it doesn't matter. If players are willing to part with the artefact, characters like Shadowheart and Astarion see this as a bit of a breach of trust.
You can ignore the Creche entirely, but you'll be missing out on some loot, and if you want to dive into Lae'zel's story, I seriously recommend not skipping it. That's because, in Baldur's Gate 3, you can visit both areas.
Travelling to the Mountain Pass and visiting Rosymorn Monastery leads you to Crèche Y'llek, where a githyanki in the Infirmary offers the procedure to remove the parasite. A character has to enter the Zaith'Isk device that purifies the host.
You can visit the creche and and then proceed to go through the underdark. You can absolutely do underdark into act 2/shadowlands and then go back to the mountain pass. I did both Creche and Underdark.
If you decide not to do Lae'zel's main quest to find the creche, she will leave unless you can persuade her to stay with you. Summary. Freeing Orpheus in Baldur's Gate 3 can result in the party becoming mind flayers, leading to a gloomy ending. Siding with the Emperor means losing a character and facing a harder Netherbrain fight. The choice in BG3 is a sacrifice, with no real winner, and all options mean losing something in return.
Vlaakith is meant to be the kind of threat that's the culmination of a high-level adventure. The "stronger than a conclave of archmages and a hundred sword masters" thing is just a bunch of hyperbole, meant to solidify the fact that she's very powerful in both martial and magical might.
